An employee was rigging a steel joist to be lifted by a crane. The joist fell and pinned the employee against the concrete ground. The employee sustained fractures to multiple ribs, his right arm, right tibia, and an open fracture to his right ankle, as well as a dislocated right hip.

An employee was helping unload steel slit coils on a slitting line, using an overhead crane with a C-hook. The coil did not fit properly onto a turnstile. Another operator went to remove the coil from the C-hook using a forklift while the employee held the C-hook steady. The forklift contacted the coil, causing it to fall from the hook. It landed on the employee s foot, breaking four bones. The incident took place during a training activity.

An employee was picking up scaffold frames when scaffold frames that were leaning against a concrete masonry unit (CMU) wall, fell onto the employee's back. The employee required surgery for a blood clot in their neck.

On September 26, 2025, an employee was performing maintenance on an excavator when a bale of rubber fell from an adjacent rack and struck him. The employee sustained multiple fractures to his pelvic area.

An employee was using a telehandler to move pallets of pellets from a pod. The employee exited the telehandler to pick up a sacrificial pallet that is placed under the bottom layer of pallets. When the employee turned to pull the pallet away, the top two tiers of the pod fell on him. The employee sustained a fractured ankle, three fractured ribs, and fractured vertebrae.
