Before the incident

On [REDACTED] at [REDACTED] [REDACTED] was operating the RTS splicer #1 when he encountered an issue with material not cutting all the way across. He attempt to resolve the issue [REDACTED] reached in without realizing that the slicer blade was stuck to the material due to a broken blade.

What happened

When he moved the material the stored energy caused the knife to continue it 's cycle resulting in contact with [REDACTED] inner forearm. [REDACTED] was promptly taken to the ER by ambulance and received medical treatment including 19 stitches.
