Before the incident

Rigging canopy frame

What happened

The team was using an all - terrain forklift to move canopy frames into a stack out of the way. They were using two 20 synthetic slings rated to 2500 lbs. in a choke capacity. The rigging method was a double chocker hitch attached to a single lifting point which was the forklift jib. The crew successfully moved and stacked 3 frames but stated that the sling kept sliding each time. While attempting to move the 4th canopy the injured employee was manually guiding the canopy while suspended. While guiding the canopy at arms length the single sling failed causing the canopy to swing towards the employee landing on his feet and causing him to fall backwards.
