Before the incident

Returning to the shop

What happened

Two Millwrights were returning to the plywood maintenance shop after finishing a repair in the merchandising area. They were both traveling in different maintenance carts. When the employee in the first cart got to the intersection of the slab he looked to his left to see where the [REDACTED] was. He continued to proceed forward when he looked up and saw a bite of logs about 10 feet in front of him. He immediately hit the brakes. The [REDACTED] did not see him until he noticed the flashing light on the front of the millwright cart. He immediately stopped. The millwright driving the cart was pushed back in the seat pinning him between the logs of the [REDACTED] and the seat of the cart. The [REDACTED] driver immediately got out to see if he was ok then got back in to back up the machine. The other [REDACTED] was also there at that time and helped him get out of the cart and made sure he was ok. The millwrights drove to the maintenance breakroom where they contacted the on-shift supervis
