free from recognized hazards that were causing or likely to cause death
or serious physical harm to employees in that employees were exposed to
the hazards of being struck by fragments of steel from blasting tank due
to the overpressurization of a non-rated 275 gallon per tank on 6/5/87
at the Franklin Lakes Becton Dickinson construction job site:
One feasible and acceptable abatement method, among others to correct
this hazard, is to use only ASME coded tanks with safety relief devices,
pressure guages and bottom drain socks.
