General-duty citation text

Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not furnish employment and a place of employment which were
free from recognized hazards that were causing or likely to cause death
or serious physical harm to employees in that employees were exposed to:
a) Employees are exposed to the hazard of falling into a 12 ft. deep
waste water tank and to the hazard of falling about 14 ft. to a
concrete surface when operating its valves to tanks 401, 402, 451
and 452 located behind Bldg. FGD. This hazard exists when employees
stand on pipes, outside the "catwalk" to operate these valves.
One possible abatement method, among others, would include preventing
the employee from leaving the "catwalk" by either extending the
"catwalk" so the employees could operate these valves wihtout having
to climb out on these pipes or by installing pneumatic/hydraulic valve
controls that could be operated from the "catwalk".
