Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not furnish employment and a place of employment which
were free from recognized hazards that were causing or likely to cause
death or serious physical harm to employees in that employees were
exposed to:
On or about 1/26/93, employees could fall and be  fatally injured
from unprotected leading roof edges 18 ft. to 21 ft. 6 in. above
concrete floors while not using fall protection and installing
steel roof decking, laying out insulation material and driving
screws.
Among others, some feasible and useful methods to correct this
hazardous condition would be, to use safety belts and lanyards
rigged as a restraint device to prevent a fall, safety harness
and lanyard, scaffolds with guard railings, and aerial lifts
or equivalent fall protection.
In that; on or about 1/26/93, employees could fall from a
scaffold plank 18 ft. to 21 ft. 6 in. above concrete floors and
be fatally injured whiel not using fall protection and rolling
out fiberglass insulation.
Among others, some feasible and useful metthods to correct this
hazardous condition would be, to use safety belts and lanyards
rigged as a restraint device to prevent a fall, safety harness
and lanyard, scaffolds with guard railings, and aerial lifts
or equivalent fall protection.
