Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not furnish employment and a place of employment which
were free from recognized hazards that were causing or likely to cause
death or serious physical harm to employees in that employees were
exposed to:
a) Employees riding in the Otis elevator car were exposed to
the hazard of uncontrolled elevator car descent due to the
employer's failure to provide annual safety tests and
inspections of elevator car safeties and, where found
defective, maintenance and/or repair of the safeties.  Among
other methods, one feasible and acceptable method to correct
this hazard is to test the safeties every twelve (12) months
as required by ANSI A17.1-1978, Rule 1001.4 a.

Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not furnish employment and a place of employment which
were free from recognized hazards that were causing or likely to cause
death or serious physical harm to employees in that employees were
exposed to:
a) Employees were exposed to falling into an elevator shaft or
being struck by a descending car.  Otis elevator hoistway
doors at the third floor and basement levels had defective
latches that permitted the doors to be opened when the car
was not at that level.  Among other methods, one feasible
and acceptable abatement method to correct this hazard is
to repair or replace all defective locks.
