Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not furnish employment and a place of employment which were
free from recognized hazards that were causing or likley to cause death
or serious physical harm to employees in that employees were exposed to
crushing hazards:
(a)  Throughout facility  -  on or about 12/24/92, employees were
exposed to crushing hazards.  In the food warehouse racks were
not provided with adequate strength and/or impact resistance
to avoid damage by forklifts, which resulted in the failure of
and collapse or racks loaded with pallets of bottled water.
Abatement Note:
One feasible and acceptable abatement method among others to
correct this hazard is:  to provide impact resistance to racks.
Options provided by manufacturer to limit forklift impact damage
(Sturdi built product guide pg. 9) and unload and reinforce
damaged racks.  Both methods are required by ANSI Standard
MH16.2('84) Section 1.3 and 3.5
