Section 5(a)(1) of the Occupational Safety and Health Act of 1970:
The employment and a place of employment which were free from
recognized hazards hat were causing or likely to cause death or
serious physical harm to employees in that employees were exposed to
a crushing hazard, electrocution, or burns.
(a)  Aboard  M/V Conception, there were no written procedures,
rules or training developed, implemented or being conducted
on energy controls for employees exposed to hazards such as
inadvertent activation of processing equipment mechanical,
electrical and engine room equipment when they work on
repairing such equipment in accordance with CFR 1910.147 and
ANSI Std Z244.1-1982, section 4 & 5.
Among other methods, one feasible and acceptable method to
correct this hazard is to develop and implement a lockout/
tagout program using the sample materials provided.
