Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The
employer did not furnish employment and a place of employment which were
free from recognized hazards that were causing or likely to cause death
or serious physical harm to employees in that employees were exposed to:
a) P/B Royal Aleutian: Employees in the upper blast freezer, starboard
side, had occasion to use a wall mounted duplex outlet that had a
broken receptacle. This condition subjected the employees to the
possibility of contacting energized electrical circuits, resulting
in shock, burns, and/or electrocution.
Among other methods, one feasible and acceptable abatement method to
correct this hazard is to replace the receptacle.
b) P/B Royal Aleutian: Employees in the upper blast freezer, starboard
side, had occasion to use shop built, gray 50 foot extension cord
which was not equipped with a grounded male plug. This condition
subjected the employees to the possibility of becoming a path to
ground, resulting in shock, burns, and/or electrocution.
Among other methods, one feasible and acceptable abatement method to
correct this hazard is to replace the male plug.
c) P/B Royal Aleutian: Employees on the forward top deck had occasion
to walk on the flexible power supply cable laying on the deck.
Approximately 20 feet of flexible power cable was not enclosed in
conduit and other required fittings. Cable supplied power for the
hydraulic pump motor oin the plywood box. This condition subjected
employees to the possibility of walking on energized wires resulting
in shock, burns, or electrocution.
Among other methods, one feasible and acceptable abatement method to
correct this hazard is to wire the equipment to meet the National
Electrical Code or Institute of Electrical and Electronics Engineers
Standards.
d) P/B Royal Aleution: Employees in the ice house, top deck, had
occasion to work near or use the incandescent light fixtures that
did not have the shielded wire secured in the strain relief fitting
in the junction boxes.
e) P/B Royal Aleution: Employees in the upper blast freezer, starboard
side, had occasion to work near or use Dayton hacksaw that did not
have the insulated wire jacket secured in the strain relief fitting
in the box.
Among other methods, one feasible and acceptable abatement method to
correct this hazard is to wire the equipment to meet the National
Electrical Code or Institute of Electrical and Electronics Engineers
Standard.
f) P/B Royal Aleution: Employees in the box up area had occasion to use
a 4-inch junction box (type with knock-outs, designed to be enclosed
in a wall) with a 100-volt duplex outlet, attached to a shop built
drop cord. This condition subjected the employees to the possibility
of touching the wet, defective junction box, resulting in shock,
burns, and/or electrocution.
Among other methods, one feasible and acceptable abatement method to
correct this hazard is to replace the 4-inch junction box with a
portable weatherproof outlet box.

a) P/B Royal Aleutian: Employees in the box up area had occasion to use
a shop built drop cord with a 4-inch junction box which was wired so
as to reverse the polarity. This condition subjected the employees to
the possibility of becoming a path to ground, resulting in shock,
burns, and/or electrocution.
Among other methods, one feasible and acceptable method to correct this
hazard is to wire the duplex receptacle to meet the National Electric
Code or Institute of Engineers and Electronic Engineers Standards.
Royal Aleutian Seafoods was previously cited for a violation of this
Occupational Safety and Health Standard or its equivalent standard 29
CFR 1910.304(a)(2) which was contained in OSHA Inspection Number
107100117, Citation Number 1, Item Number 1, issued on 03/20/89.
