Section 5(a)(1) of the Occupational Safety and Health Act of 1970:
On or about 3/3/93, the employer did not furnish employment and a
place of employment which were free from recognized hazards that were
causing or likely to cause death or serious physical harm to employees
in that employees were exposed to the hazards of confined space entry
when entering reactor vessels that contained residue from various
chemical products which had been mixed in the reactor:
a- William Zinsser and Co., Polymer (Zinchem) Dept. - Employees
were required to enter 3 glass-lined reactor vessels in order
to inspect the interior and to scrape polymer "skins" off the
interior walls and bottom of the vessel.  There are two 2,000
gallon vessels and one 1,000 gallon vessel, with reactor
manholes ranging from 18 inches to 28 inches.  The depth of
the reactors ranged from 7 feet deep to 13 feet deep.
The employer did not establish an effective written permit
system to certify that all hazards or potential hazards hav
ebeen evaluated.  The employer did not provided training to
vessel entrants. attendants and entry supervisors in order tha
employees may safely work in the vessels.  The employer did
not provide or ensure the proper use of equipment (ventilation,
testing, personal prtective equipment and rescue) necessary
for safe entry.  The employer did nto ensure that procedures
and personnel necessary to rescue employees from the vessels
were provided and implemented.
Condition noted on or about 3/3/93.
Among others, one feasible and acceptable abatement methods
to correct this hazard would be to implement the elements of
a permit-required confined space program as found in 29 CFR
1910.146(d).
