hazards that were causing or likely to
cause death or serious physical harm to employees in that they were
exposed
to struck-by or crushed-by hazards.
Job site at 1202 Madison Hwy, Valdosta, GA - A fork lift operator was
observed driving a Taylor industrial fork
lift (Model TEB250E) without wearing the supplied seat belt as required by
ANSI B56.1a-1989, paragraph 5.3.9,
and the load was being transported while elevated, rather than being
lowered.  This was exposing the employee to
the hazards of being struck / crushed by the lift truck overhead
protection device, should the fork lift be accidentally
tipped over and result in the driver being thrown out of the operator
seat, on or about 10/08/97.
One feasible and acceptable abatement method to correct this hazard would
be:  To ensure that drivers wear supplied
seat belts anytime they operate industrial fork lift trucks and lower the
loads to the lowest possible position while
traveling.
