Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not furnish employment and a place of employment which
were free from recognized hazards that were causing or likely to cause
death or serious physical harm to employees in that employees were
exposed to the hazard of electrical shock:
FARM:  A welding machine was being used to do maintenance work on
"A BIN" for coup 37.  The cord on the welding machine was spliced
and loosely taped.  The cord was plugged into a short extension cord
(pigtail) that was temporarily wired into a circuit breaker panel.
The pigtail extension cord was ungrounded.  The cover was removed
from the circuit breaker panel by plant personnel in order to attach
the pigtail extension cord.
Among other methods, one feasible and acceptable abatement method
to correct this hazard is:
1.  Replace the spliced cord on the welder.
2.  Institute a preventive maintenance program and periodic inspection
program to ensure that the cords on all portable equipment are
maintained in safe condition.
3.  Install permanent grounded outlets so that pigtail extension cords
do not need to be temporarily wired into circuit breaker panels.
4.  Ensure that only qualified electricians work on live electrical
circuits or remove covers from live electrical panels.
5.  Institute electrical safe work practice rules for the company
to ensure that whenever employees could be exposed to live electrical
parts the circuits are locked out or other precautions are taken to
isolate employees from the hazard of live parts.  See National Fire
Protection Standard NFPA 70E for electrical safe workpractice
guidelines to follow.

exposed to the hazard of traumatic injuries from broken machine
components:
FARM:  An employee was operating a vehicle mounted elevating and
rotating aerial device, Asplundh Line Lift Model L36, mounted on
Chevrolet stake body truck CE538T110885.  He was using it for access
to do work on the top of "A BIN" for coup 37.  The fiberglass portion
of the boom broke.
Among other methods, one feasible and acceptable abatement method
to correct this hazard is:
Review American National Standards Institute standard A92.2 entitled
"Vehicle Mounted Elevating and Rotating Aerial Devices" and the
Asplundh Operation and Maintenance manual for this machine.  Based
on these two documents develop an inspection and testing program that
provides for initial inspections and tests, frequent daily inspections
and tests as well as periodic thorough inspections and tests of the
machine.  Maintain appropriate preventive maintenance, inspection
and test records on the machine and on the truck.

exposed to the hazard of falls:
FARM:  An employee operating a vehicle mounted elevating and rotating
aerial device, Asplundh Line Lift Model L36, mounted on Chevrolet
stake body truck CE538T110885 was not wearing a safety belt and
lifeline attached to the boom to keep him from falling.
Among other methods, one feasible and acceptable abatement method
to correct this hazard is to:
Provide a safety belt and lifeline for use by operators while elevated
in the bucket of the aerial lift.  Instruct operators in the proper
and safe use of the safety belts.
