Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not furnish employment and a place of employment which
were free from recognized hazards that were causing or likely to cause
death or serious physical harm to employees in that employees were
exposed to:
The hazards of impalement and death from waling into, tripping
on, and falling onto pipe stub protrusions 1/2 in. diameter to
1 in. diameter by 12 in. high in the concrete floor and which
were not guarded, located in the fuel deck area and in the
restaurant area.
Among other methods, one feasible and acceptable abatement method
to correct this hazard is to cap the pipe ends with covers such
as used over rebar.
