Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not furnish employment and a place of employment which
were free from recognized hazards that were causing or likely to cause
death or serious physical harm to employees in that employees were
exposed to:
Fractures, crushing injuries, or death from being struck by or
crushed by falling molds being installed in plastic injection
molding machines.  Molds, such as Whirlpool tool No. 62941-SD-5
with a listed weight of 6,800 pounds were lifted and moved with
hand-chain powered hoists with a total rated bridge capacity of
3 tons (6000 pounds) which resulted in the overloading of the
bridge and the possibility of failure and of dropping the mold
onto workers.
Among other methods, one feasible and acceptable method to
correct this hazard is to have the bridge of the hoist units
strengthened in order to increase the rated capacity to handle
all loads which it may be expected to handle.
