Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not furnish employment and a place of employment which
were free from recognized hazards that were causing or likely to cause
death or serious physical harm to employees in that employees were
exposed to multiple injuries from falling hazard when employees are
elevated on platforms when used with a powered industrial truck and
the platform is not secured to the lifting carriage and/or forks and
not provided with a midrail:
(a) Maintenance Department; the two (2) personnel
platforms were not secured to the powered industrial
trucks lifting carriage and/or forks and did not have
a guardrail system that included a midrail.
Among other methods, one feasible and acceptable abatement method to
correct this hazard is to secure the personnel platform to the lifting
carraige and/or forks and install a midrail in the guardrail system.

Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not furnish employment and a place of employment which
were free from recognized hazards that were causing or likely to cause
death or serious physical harm to employees in that employees were
exposed to crushing and amputation injuries during the operation of
the presses provided with inadequate barrier guards, improperly used
two hand control devices and/or improperly adjusted presence-sensing
devices:
(a) Sizing Department; the part revolution mechanical
power presses #L-106,L-92, L-104, 204, 232, 212, 52,
197, and 161 had two hand controls that were activated
with the operators elbows which allowed the hands
to be within the safe distance established for the
hands to be used to activate the presses.
(b) Molding Department; the full revolution mechanical
power press #61 had an opening in the top of the
barrier guard approximately 26" long and 8" wide and
the guard was readily removable by the operator.
(c) Molding Department; the full revolution mechanical
power press #L-95 had an opening in the front of the
barrier guard approximately 5 1/2" high and the guard
was held shut by a slide latch that was readily
movable by the operator.
(d) Molding Department; hydraulic power press #201
did not have the light curtain properly adjusted in
that approximatelya 5" high opening existed across
the front of the die bed.
Among other methods, feasible and acceptable abatement methods to
correct these hazards include:
(1) Only allow the hands to be used to activate the
controls.
(2) Provide point of operation guards that prevent
entry of hands or fingers into the point of operation
by reaching through, over, under, or around the guard,
and utilize fasteners that are not readily removable
by the operator.

exposed to crushing and amputation injuries from the failure of the
brake during the feeding or removing of parts into the presses while
the presses were guarded at the point of operation by two hand control
devices:
(a) Sizing Department; the part revolution mechanical
power presses L-104 and 142 operated by two hand
control controls with parts either placed into the
point of operation or removed manually.
Among other methods, one feasible and acceptable abatement method to
correct this hazard is to install a brake monitor.

employer did not furnish employment and a place of employment which
were free from recognized hazards that were causing or likely to cause
death or serious physical harm to employees in that employees were
exposed to crushing and amputation injuries during the operation of
the presses in that there were not routine inspections to identify
hazards with the presses and operator and/or supervisors were not
adequately trained to recognize hazards associated witih safe press
operations:
(a) Molding Department; power presses were being
operated with inadequate guarding.
(b) Sizing Department; power presses were being used
with inadequate guarding.
Among other methods, feasible and acceptable abatement methods to
correct these hazards include:
(1) Require that the individual assigned the
responsibility for tool setting ensure that
appropriate safeguarding is installed.
(2) Ensure the original and continuing competence
of personnel servicing, inspecting and maintaining
presses.
(3)  Ensure that these employees involved in press
set-up and operation have training and information
to set-up and operate presses safely and ensure that
these instructions are followed.
(4) Ensure that all employees associated with presses
are instructed so they can perform the functions for
which they are responsible.
(5) Establish and follow a program of periodic and
regular inspections of presses to ensure all their
parts, auxillary equipment and safeguards are in safe
operating condition and adjustment and maintain a
certification record of inspections.
(6) Inspect and test each press no less than weekly
to determine the condition of the clutch/brake
mechanism, anti-repeat feature and single stroke
mechanism, and maintain a certification record of
tests, inspections and maintenance.
