Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to the hazard of being struck, crushed or pinned by unsecured
automotive lifts;
e.g. the automatic mechanical locking latches were not engaged:
a)On or about 10-5-94, North side of shop, (Alignment Rack); Dover (Rotary
lift)
semi hydraulic double cylinder automotive lift, serial number 178049,
model number
JG166; employees were exposed to the hazard of being struck, crushed or
pinned while
working under vehicles on the lift; automatic mechanical locking latches
were
inoperative.
b)On or about 10-5-94, North side of shop, (Ed); Dover (Rotary lift) semi
hydraulic
double cylinder automotive lift, model number JG620-C, no identification
tag;
employees
were exposed to the hazard of being struck, crushed or pinned while
working under
vehicles on the lift; automatic mechanical locking latches were
inoperative.
c)On or about 10-5-94, North side of shop, (Ed); Dover (Rotary lift) semi
hydraulic
single cylinder automotive lift, serial number 176445, model number
illegible; employees
were exposed to the hazard of being struck, crushed or pinned while
working under
vehicles on the lift; automatic mechanical locking latches were
inoperative.
d)On or about 10-5-94, North side of shop, (Brian); Dover (Rotary lift)
semi
hydraulic double cylinder automotive lift, model number JG620-C, no
identification tag;
employees were exposed to the hazard of being struck, crushed or pinned
while working
under vehicles on the lift; automatic mechanical locking latches were
inoperative.
e)On or about 10-5-94, North side of shop, (Brian); Dover (Rotary lift)

hydraulic double cylinder automotive lift, serial number 176444, Model

employees were exposed to the hazard of being struck, crushed or pinned
while working
under vehicles on the lift; automatic mechanical locking latches were
inoperative.

number JG163A;
employees were exposed to the hazard of being struck, crushed or pinned
while working
under vehicles on the lift; automatic mechanical locking latches were
inoperative.

hydraulic double cylinder automotive lift, No identification tag;
employees were exposed
to the hazard of being struck, crushed or pinned while working under
vehicles on the lift;
automatic mechanical locking latches were inoperative.
h)On or about 10-5-94, North side of shop, (Jeff); Dover (Rotary lift) semi
hydraulic double cylinder automotive lift, model number JG620C; employees
were
exposed to the hazard of being struck, crushed or pinned while working
under vehicles
on the lift; automatic mechanical locking latches were inoperative.i)On or
about 10-5-94, South side of shop, (Dave); Dover (Rotary lift) semi
hydraulic double cylinder automotive lift, number 837440; employees were
exposed to
the hazard of being struck, crushed or pinned while working under vehicles
on the lift;

were exposed

hydraulic double cylinder automotive lift, No identification tag;
employees were exposed
to the hazard of being struck, crushed or pinned while working under
vehicles on the lift;
automatic mechanical locking latches were inoperative.
Among other methods, one feasible and acceptable abatement method to
correct this hazard is
to:
a) Conduct inspection of the lifts as recommended by the manufacturer to
ensure that the
automatic mechanical locking latch and all other operating components are
in safe operating
condition.  Remove the defective lift(s) from service until rendered safe
for use.
b) Follow the requirements of ANSI B153.1-1990, Automatic Lift Institute
Publication ALISM
89-1 and NIOSH Publication No. 77-22.  The Automatic  Mechanical Locking
devices shall hold
the lift in the extended position.
c) Instruct and train employees as recommended in the manufacturer's
procedures for proper use
in the safe operation of the lifts, including the automatic mechanical
locking latch.g
