Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employmnent and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to the hazard of dropped mold plates possibly falling on an
employee:
(a) Perryville, Missouri location in Building One in the Press Mold Area;
Makeshift slings made from common steel chain with welded end attachments
and makeshift rigging attachments were attached with bolts to mold plates
and
used for hoisting.  The slings, attachments and bolts had not been tested
or
certified for the purpose.
While ultimate responsibility rests with the employer, given his/her
superior knowledge of
the workplace, one feasible and acceptable method to correct this hazard,
among others,
includes providing employees with suitable slings and rigging equipment,
as referenced in
Section 16-1.3.2 of ANSI B30.16-1993, "American National Standard for
Overhead Hoists".
Essential elements include:
1.  Provide slings and attachments which are manufactured and rated
appropriately for the loading requirements of their intended use.
2.  Periodically inspect all hoisting and rigging equipment to assure that
it
is not damaged, excessively worn or defective; and maintain any
required records.
3.  Remove any hoisting or rigging equipment from service which is not
acceptable for use.
