Section 5(a)(1) of the Occupational Safety and Health Act of 1970:
The employer did not furnish employment and a place of employment
which were free from recognized hazards that were causing or likely
to cause death or serious physical harm to employees in that,
employees were placed into positions as a multicraftsmen without
being provided training.  The equipment tenders positions were a
combination of a saturator operator position and a pipefitter
repairman position; and an oil stillmen operator and a pipefitter
repairman.  The employees that were placed in these positions were
pipefitter repairmen and were not provided the knowledge and skills
to perform as operators.
AMONG OTHER METHODS, ONE FEASIBLE AND ACCEPTABLE ABATEMENT METHOD TO
CORRECT THIS HAZARD IS to provide training to the equipment tenders
that includes, but is not limited to, the operating procedures for
the departments in which they were assigned.

the operator to respond
to all possible emergencies such as but not limited to, loss of feedwater,
loss of boiler, and
loss of pump all at the same time.  Controls and instrumentation for the
boiler are located in
various locations throughout the four story boiler house.
AMONG OTHER METHODS, ONE FEASIBLE AND ACCEPTABLE ABATEMENT
METHOD TO CORRECT THIS HAZARD IS to provide central locations for all
instrumentation and controls for the boiler functions so the operator will
be able to
continually monitor and control them as different situations develop.
