Section 5(a)(1) of the Occupational Safety and Health Act of 1970:
The employer did not furnish employment and a place of employment which
were free from
recognized hazards that were causing or likely to cause death or serious
physical harm to
employees in that employees were exposed to crushing injuries, in the
Automotive Express
Center, due to:
(a)The lift control mechanism for the #2 dual post, hydraulic lift, was not
installed properly in that it would make contact with the block wall, when
released by the technician, and allow the lift to continue to rise.
(b)Automotive Express Center.  Periodic lift inspections did not ensure
that the
equipment was kept in safe condition in that a small piece of metal, which
limits the movement of the car support arm, was missing on the #2 dual post
hydraulic lift and should have been noticed during the daily, weekly, or
monthly inspections.
AMONG OTHER METHODS, ONE FEASIBLE AND ACCEPTABLE ABATEMENT
METHOD TO CORRECT THESE HAZARDS IS to:
(a)Properly install the lift control mechanism, in accordance with the
American
National Standard ANSI B153.1-1974, Section 3.1.3, so that it automatically
returns to the neutral or "off" position when it is released by the
technician.
(b)Repair the car support arm by welding a piece of metal back into place
and
conduct thorough periodic inspections of the lift, in accordance with the
American National Standard ANSI B153.1-1974, Section 6.3, to ensure that
all
components are in safe operating condition.
