Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to a crushing injury from falling basket(s) and tubing:
a) Employees use wire rope slings to hoist baskets containing tubing
weighing
approximately 1000 pounds (the basket and tubing weight). The wire rope
slings are
connected with one U-bolt clip on each leg and no thimble is used in the
eyes to maintain
rope strength and reduce wear, thereby reducing the safe working load of
the sling.
ABATEMENT NOTE: One feasible method to correct this item, among others,
is to install the
wire rope clips according to established rigging practice and incorporate
rope thimbles in the
eye.
OR IN THE ALTERNATIVE:

a) Employees use defective wire rope slings to hoist baskets containing
tubing weighing
approximately 1000 pounds (the basket and tubing weight). The wire rope
slings are
connected with one U-bolt clip on each leg and no thimble is used in the
eyes to maintain
rope strength and reduce wear, thereby reducing the safe working load of
the sling.
