Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to:
a) Procedures for safety unplugging the down commers were not developed and
effectively communicated to employees potentially exposed to hot fly ash
when
unplugging lines from the economizer fly ash hoppers to the fly ash
economizers 1E1 and
1E2.
b) Employees unplugging down commers at economizers 1E1 and 1E2 and
exposed to
falls into economizers, and being struck by the metal access cover or hot
fly ash when
the cover "blows off".
Among other methods one feasible and acceptable abatement method to
correct this hazards:
a)1. Develop and document procedures for safely unplugging the down
commers and train employees accordingly.
These procedures should include provisions for hold landing the fly ash
hopper knife values when employees are working in or near the
economizers and release of fly ash is possible, and the wearing of
appropriate PPE.
2. Change economizers 1E1 and 1E2 to a dry system similar to
economizers 2E1 and 2E2.
b) Ensure that the economizer tank access corners to both 1E1 and 1E2
are bolted down completely as
