Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to fall and contact hazards from defective elevator doors: (a). Elevator Area-basement - The safety interlock for the elevator door was by-passed, using a block of wood wedged under the latch. The elevator could be operated without the doors closed and could create a hazard of either falling though the opening, or contacting the building structure during movement of the car.
