Section 6(a) of the Puerto Rico Occupational Safety and Health Act:  The
employer did not
furnish employment and place of employment which were free from recognized
hazards that
were causing or likely to cause death or physical harm to employees in
that employees were
exposed to:
Seccion 6(a) de la Ley de Seguridad y Salud en el Trabajo de Puerto Rico:
El patrono no
proveyo empleo y un sitio de empleo libre de riesgos reconocidos que esten
causando o que
puedan causar muerte o dano fisico a los empleados, por cuanto empleados
estaban expuesto
a:
In the waiting room where the patients to be evaluated wait:  By where the
employees
transit, a leak was observed in the roof and in rainy days the water go
down the wall
near and behind the switchboard that do not have the trademark and that is
at 80.25
inches high from floor level and whose dimensions are 11.5 inches high, 9
inches
long and 4.5 inches wide; also the water can go down by an electric cable
that exits
the switchboard and go down to the floor, the high from the cable exit to
the floor is
84 inches.
Hazard:  In rainy days, slips, falls and contact with electrical energy.
En la sala de espera donde los pacientes esperan para ser evaluados:  Por
donde
transitan los empleados, se observo una filtracion en el techo por donde
en dias
lluviosos baja el agua por la pared por el lado y por detras de una caja de
interruptores electricos que no tenia marca y que esta a 80.25 pulgadas de
alto desde
el nivel del piso y cuyas dimensiones son 11.5 pulgadas de alto, 9
pulgadas de largo y
4.5 pulgadas de ancho; tambien puede bajar agua por un cable electrico que
sale de la
caja de interruptores electricos y que llega hasta el piso, la altura
desde la salida del
cable hasta el piso es de 84 pulgadas.
Riesgo:  En dias lluviosos, resbalones, caidas y contacto con energia
electrica.

In the disbursement area:  Where the payers give the vouchers to the
patients were
observed two leak spots and because of this in rainy days the floor can be
wet.
a)  A leak spot in the roof near the air conditioner conduct.
b)  A leak spot in the west side wall.
Hazard:  In rainy days, an employee can slip or fall.
En el area de los pagadores:  Donde los pagadores le hacen entrega de los
"vouchers"
a los pacientes, se observaron dos manchas de una filtraciones, por donde
en dias
lluviosos se puede mojar el piso.
a)  Una mancha de filtracion en el techo al lado del conducto del aire
acondicionado.
b)  Una mancha de filtracion en la pared del lado oeste.
Riesgo:  En dias lluviosos, algun empleado se puede resbalar o caerse.r
