Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not furnish employment and a place of employment which
were free from recognized hazards that were causing or likely to cause
death or serious physical harm to employees in that employees were
exposed to:
a)  Basement, First & Second Floor - La Cross Div.
Employees utilizing the plant "Dumbwaiter", to transport company
products from one to any of three industrial plant floor levels are
exposed to serious injuries from ascending or descending hoist when
hoist way door(s) were opened.  Among other feasable and acceptable
method(s) of abatement one method of abatement is to install
electrical contacts, interlocked with the hoistway system, at all
three levels, preventing any door from opening, unless the hoist is
at the level called from.
Reference ANSI/ASME A17.1-1981, Part VII, "Hand and Power
Dumbwaiters" Section 700, Rule 700.10a, "Hoistway-Door locking Devices
for Power Dumbwaiters",  conforming to the ANSI requirements of Rules
111.4C, 111.4d, 116.6 and 111.7b.
