Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The
employer did not
furnish employment and a place of employment which were free from
recognized hazards
that were causing or are likely to cause death or serious physical harm to
his employees in
that employees were exposed to:
(1) Frigoscandia spiral conveyor system, hazard of being caught in pinch
points
created by the rotating cage spoke member of the spiral conveyor and
stationary compression
tube member.
(2) Frigoscandia spiral conveyor system, hazard of being struck by the
rotating cage
spoke member of the spiral conveyor.
At the job site located in the Kraft Foods Inc., Tombstone Pizza plant in
Medford,
Wisconsin on or about 23 September, 2001, employees of KVP Falcon Plastic
Belting Inc.
were installing plastic conveyor belting on the Frigoscandia spiral
conveyor
while the
conveyor was in motion.  During this activity at least one employee was
inside the revolving
structure of the conveyor while it was in motion and the conveyor's
operation was being
controlled by another employee outside the structure, who was not always
in direct visual
contact with the employee on the inside.
A feasible and acceptable means of abatement would be to prohibit
employees from
entering the drum of the spiral conveyor when it is in motion.  The
American Society of
Mechanical Engineer's ANSI B20.1-2000 Safety Standard for Conveyors and
Related
Equipment: Prohibit the practice of riding in/on the conveyor during belt
installation.  Riding
on conveyors not specifically designed for riding is prohibited by Section
5.12(d).
(1) Establish a program and procedure to install belts on a spiral
conveyor without
having to go inside the cage and climb the structural members while the
spiral conveyor is in
motion. Included in the written program should be a job hazard analysis
that identifies all the
hazards that are associated with the installation of belts on a spiral
conveyor.
(2) Establish a training program to train employees in the installation of
belts on
spiral conveyors without having to go inside the spiral conveyor to "RIDE"
the conveyor
during the installation process.
