Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: Being struck by the load should the jib monorail assemblies or underhung chain hoists fail: (a) In the manufacturing building; four jib type monorails with attached underhung chain hoists are supported from two steel and concrete filled posts ( two monorails per post). These monorail and post assemblies which were fabricated in house and carry no labeling of load rating, it was unknown what loads these assemblies could carry thus posing a crushing hazard to employees if they were to fail. (b) In the manufacturing, repair, and paint buildings; underhung electrical and hand operated chain hoists such as but not limited to two Wrightway 2000 pound, one CM 1/2 Ton, and one Lodestar chain hoist, no complete program of frequent or periodic inspections has been implemented. Among other methods, feasible and acceptable methods to correct these hazards are: (a) Determine the load rating of the two jib monorail and post assemblies, ensuring that they are at least equal to the rating of the maximum capacity of the underhung hoists, or provide a structure with such rating, as per ANSI B30.16-1.3.2. (b) Perform frequent inspections at daily to monthly intervals and periodic inspections at monthly to yearly intervals as per ANSI B30.16-2.1, 2.1.2., 2.1.3., or the owners manuals for all overhead hoists in your facility. Written, dated and signed inspection reports should be maintained at time intervals specified in ANSI B30.16-2.1.1(b)(2).
