Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazard of being struck by a hose assembly that becomes dislodged during operation: a)SCRUBBER #1, HOPPER ACCESS PLATFORM, on or about 07/17/97: Hose assembly used to connect "Air Cannon" pressure tank to lower south side clean-out port, on Scrubber #1, was fitted with a 4-inch diameter cam & groove coupling which per manufacturer, are not designed for use with compressed air, but for use with liquids. Hose assembly was being subjected to 100 psi air pressure, but the polypropylene male connector (lower south side clean-out port) was rated per manufacturer at 50 psi. While this hose assembly was being used, it dislodged from the lower south side clean-out port and struck an employee. Abatement Notes: Among other methods, one feasible and acceptable abatement method to correct this hazard is to: Follow the manufacturer's recommendations for couplings and connectors for use with compressed air at the required pressure for these devices. Abatement documentation must be submitted for this Citation.

Hose assembly used to connect "Air Cannon" pressure tank to lower south side clean-out port, on Scrubber #1, was fitted with a 4-inch diameter Dixon cam & groove coupling which had visible cracks and the compression gasket was missing. Abatement Notes: Among other methods, one feasible and acceptable abatement method to correct this hazard is to: Develop and implement written procedures for installation, operation and maintenance of hose assemblies and associated couplings as recommended by hose/coupling manufacturers. Abatement documentation must be submitted for this Citation.
