Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not furnish
employment and a place of employment which were free from recognized
hazards that were causing or
likely to cause death or serious physical harm to employees in that
employees were exposed to the
following conditions:
(a)On October 29, 1998, at approx 5:30 pm at a gas well located 1/4 mile
from FM 3454
in Riverside, Texas 77367 where an employee was hit by an unsecured
return/discharge
line piping system and suffered a fatal injury.  The pipes were located on
top of the mud
tanks and during a power surge came loose at the elbow and the pipe swung
outward and
up hitting the employee in the head.
Among other methods, feasible and acceptable methods of abatement to
correct the
hazards are to:
1.Develop a comprehensive written program and checklist using the
American Petroleum Institute RP54-1981.  Recommended Practices for
Occupational Safety and Health for Oil and Gas Well Drilling and
Servicing Operations.
2.Provide a positive means of securing all pressure lines, piping system on
the mud tank, and connections with safety chains, wire rope or "U" bolts
during the drilling operations as required by paragraph 8.14.3 of RP 54
Recommended Practices for Occupational Safety and Health for Oil and
Gas Well Drilling and Servicing Operations.
3.Ensure that such means are properly installed following drilling rig
setup
and before the mud pump is placed into operation.
