Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to the hazard of fire and/or explosion:
a)King Salmon Platform, well # K-17:   On or about April 20, 2002, and at
times prior
there to, well control was not maintained at all times as required by
American Petroleum
Institute API 54 Third Edition, August 1999 Recommended Practice for
Occupational
Safety for Oil and Gas Well Drilling and Servicing Operations, Section
6.1.1, and Alaska
Oil and Gas Conservation Commission Regulation 20 AAC 25.033(a).  While
performing
ESP well work-over procedures, a well control event occurred that resulted
in an
uncontrolled release of gas and liquid hydrocarbons (oil). The
hydrocarbons ignited
resulting in a fire and injuries to personnel.
Feasible and acceptable abatement methods, among others, to correct this
hazard are to establish
a well control safety program that will insure Nabors Alaska Drilling,
Inc. ("employer") will
require all toolpushers and drillers to shut in any well, or take other
appropriate action, if in
their opinion: (a) well control procedures do not conform to industry
accepted standards; (b) a
well control situation is occurring; or (c) the operator is not complying
with operations consistent
with good oilfield practices. The well control safety program shall
include regularly conducted
well control drills.
