40.1-51.1.(a):  The employer did not furnish employment and a place of
employment which were free from recognized hazards that were causing or
likely
to cause death or serious physical harm to employees in that employees were
exposed to:
(a) In the ornamental shop, an employee, operating the Eagle bending
machine,
Model #CPH 60, SN 98B0066, had not been trained in the safe precautionary
measures as outlined by the manufacturer's instructions manual.
On 8/31/99 an employee had his right index finger crushed when it was
caught
between the roller and 20 foot piece of pipe he was bending.
"One feasible and acceptable abatement method, among others, to correct
this
hazard is to train the authorized operators and set-up the bending machine
in
accordance with the manufacturer's instructions manual.tions
