Minn. Stat. 182.653 subd. 2:  The employer did not furnish to each
employee, conditions of
employment and a place of employment free from recognized hazards which
caused or were
likely to cause death or serious injury to employees; specifically:
The 12' x 12' x 2' storage rack loaded with conduit and erected on July 8,
1997 was not
erected according to manufacturers specifications nor was the rack's load
and
configuration limitations determined by a qualified person.
a) Racks were not anchored to the floor to prevent overturning.
b) Employees were not provided training to assure safe installation and
