Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to:
The recognized hazard of falling approximately ten feet from the open end
of the platform
attached to the forks of the Skytrack Rough Terrain Forklift.  On or about
June 18, 1999, an
employee fell from the platform while working at a construction site
located at 5924 S.
Sunnylane, Del City, Oklahoma.  The employee suffered injuries which
resulted in his death.
Among other methods, one feasible and acceptable method to correct this
hazard is to install a
guardrail system at the open ends of the platform.
