Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to crushing hazards.  Employee(s) that operated the Bobcat
forklift, with an
overhead guard, did not use/wear the installed seat belts..
a.  Outside Yard Area --- On or about 07/21/1999, at about 11:15 AM, an
employee
operating Bobcat forklift, serial number 5030 M 15024, model number 2000,
equipped
with an overhead guard and seatbelts, did not wear the installed seat
belts when operating
the forklift.  One feasible and acceptable abatement method, among others,
to correct this
hazard would be to have operators wear the seat belts.ors
