Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to hazard(s)of being struck by collapsing structural steel
members due to the
unauthorized modifications made to the support columns' base plates and
installation procedures
due to incorrect placement and design of the column footings. One method
of abatement to
correct this hazard is to obtain a field modification approval from the
project structural engineer
of record or building manufacture. In addition the employer shall ensure
that employees
preforming the approved modifications are adequately trained in the work
changes and are fully
supervised during the completion of all modification work.
