Section 6(a) of the Puerto Rico Occupational Safety and Health Act:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or physical harm to employees in
that employees were
exposed to:
Seccion 6(a) de la Ley de Seguridad y Salud en el Trabajo de Puerto Rico:
El patrono no
proveyo empleo y un sitio de empleo libre de riesgos reconocidos que esten
causando o que
puedan causar muerte o dano fisico a los empleados, por cuanto empleados
estaban expuestos
a:
a.  Receive serious injuries in hands at pinch point due to deactivation
of the limit switch
from access door #2 in machine trademark Hirano Tecssed Co. LTD, Model R-HC
(Recognize as CS-02). The pinch point was approximately 50" from the floor
and the
rollers had approximately the following sizes: a. Superior roller, 3.5"
diameter
by 15"
long,  b. Intermediate roller 11.5 diameter by 15" long. The machine was
located in a
conversion area from advantage, west side. Among other methods, one
feasible and
acceptable abatement method to correct this hazard is to maintain the
limit switch in
function all the time.
a.  Recibir lesiones seria en manos en punto de pinchada dado a
desactivacion del "limit
switch" de la puerta de acceso #2 en maquina marca Hirano Tecssed Co. LTD,
Modelo
R-HC (Reconocida como CS-02). El punto de pinchada estaba a 50" del nivel
del piso
aproximadamente y los rolos tenian aproximadamente los siguientes tamanos:
a. Rolo
superior, 3.5" diametro por 15" largo,  b. Rolo intermedio, 11.5 diametro
por 15" largo.
La maquina estaba localizada en el area de conversion de "Advantage", lado
oeste. Entre
otros metodos, un metodo aceptable y posible para corregir este riesgo es
mantener
el
"limit switch" funcionando en todo momento.
