Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed injuries from a pressurized air explosion of a dry bulk
transport trailer:
(a) Location - at Aggregate Industries, Everett, MA:
On or about 4/4/01, the employer did not ensure that the pressure relief
devices on the
blower used to pressurize the tanker for delivery nor the two devices on
the tank trailer
were in proper operating condition.
Among other means, one feasible means of abatement would be to:
1. Follow the blower manufacturer's instructions for inspecting and
testing the blower pressure
relief devices on a monthly basis as outlined in the Installation,
Operation and Maintenance
Manual, and
2. Follow the recommendatons for Relief Valve Maintenance in the Truck
Trailer Manufacturers
Association Technical Bulletin No. 83, Pneumatic Tank Vehicle Usage.e
