Nevada Revised Statute 618.375(1):  The employer did not furnish
employment and a place of
employment which was free from recognized hazards that were causing or
likely to cause death
or serious physical harm to employees:
a.The employer failed to ensure that employees used safety harnesses and
lanyards
anchored to approved fixed attachments points while working from a aerial
lift.
Two employees in an aerial lift, Terex TB60, serial 00630178,
approximately 16
feet high, were not wearing harnesses and lanyards, while doing patch work
on
the north wall of the Save On Drugs Store.  The employees were exposed to
possible serous injuries and death.

ANSI A92.5-1992 / 8.8(g) - Ensure all personnel in the platform are
wearing fall
protection devices and other safety gear as required at all times.
One feasible and acceptable abatement method is to make sure that
supervisor's ensure
employee's utilize fall protection when assigned to work from the aerial
lifts, prior to beginning
work.
