Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to falling loads and/or falling hoist or crane parts: (a)Fabrication Shop - On or about February 8, 2002, the underhung crane systems had the following deficiencies: 1)The employer added approximately 8 foot long cantilevered extensions to each end of the crane bridge on the Acco 5 ton underhung crane, used to move truck bodies. After this alteration, the employer did not have a rated load test done and a written record of this test prepared confirming the load rating of the crane system. 2)On the 2 ton hand pushed underhung crane system, used to move metal plates, the employer added a cantilevered extension to the end of the crane bridge opposite the metal plate storage area. After this alteration, the employer did not have a rated load test done and a written record of this test prepared confirming the load rating of the crane system. 3)The employer did not have the manufacturer's name plainly marked on the crane bridge of the 2 ton hand pushed underhung crane system, used to move metal plates. 4)The wire ropes had kinks, on the Wright 5 ton hoist on the Acco 5 ton underhung crane used to move truck bodies. 5)The latch had broken on the hook on the Wright 5 ton hoist on the Acco 5 ton underhung crane used to move truck bodies. 6)The employer did not do frequent and periodic inspections and document the period inspections on the Acco 5 ton underhung crane, used to move truck bodies, and on the 2 ton hand pushed underhung crane system, used to move metal plates. Among other methods, one feasible and acceptable abatement method to correct this hazard includes developing a frequent and periodic inspection and preventative maintenance program which incorporates all recommendations outlined in the manufacturer's manuals with written, signed and dated reports, and which follows the American National Standard Institute safety standard for Monorail Systems and Underhung Cranes (ANSI B30.11-1973), to ensure safe and properly functioning underhung crane systems.g
