Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employees did not utilized
the proper procedures resulting in four employees exposed to Sodium
Sulfide and one being struck by
the opening  of the man-way. The employees did not utilize Policy and
Procedures. The employees and
management open the man-way of a rotating dryer releasing sodium sulfide
without a proper permit. The
employees were not properly protected from the
hazards.
Among other methods, one feasible and acceptable abatement method to
correct the hazard is
ensure that the employees utilized the Company's Safe Work Permits. Ensure
that the employees
are trained and understand those procedures.The Company's Policy clearly
states that no work
on the line or equipment opening until the equipment owner has issue a
proper permit. It
provided Minimum Protective Equipment and Special Safety Protective
Equipment (safety
glasses, chemical goggles, full coverage suits to include hoods). It also
states to treat opening
lines or equipment as though they were under pressure and always wear
appropriate PPE. It
further states when breaking lines to place canvas or plastic shield over
flange or line to safe
guard against spraying or splashing of hazardous materials.
