Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The
employer failed to
provide a place of employment which was free from recognized hazards that
were likely to
cause death or serious physical harm to employees:
a. On or about August 28, 2001, at the workplace located at 2705 Techny
Road in
Northbrook, Illinois, an employee working from a Genie personnel lift was
exposed to
injuries from falls due to the lack of extended outriggers as required by
the American
National Standards Institute's standard for manually propelled elevating
aerial platforms.

Abatement documentation is required for this item in accordance with the
requirements of 29
CFR 1903.19(d).
