North Carolina General Statute 95-129(1) of the Occupational Safety and Health Act of North Carolina: The employer did not furnish to each of his employees conditions of employment and a place of employment free from recognized hazards that were causing or likely to cause death or serious physical harm to employees, in that employees were exposed to conveyer start up operations without continuous warning alarms along the length of the conveyer: Reference ANSI/ASME B20.1 - 1990 sec. 5.11.2 (b)(1) a)facility - Bridgestone/Firestone did not provide audible warning devices that could be clearly heard at all points along the conveyor during the start up operations.
