Elevators and Escalators (ASME A17.1-2000).  Elevator hoistway had access
openings that were not locked or closed.
Among feasible abatement methods, the elevator hoistway should have access
openings
designed in accordance with section 2.7. of the ASME A17.1-2000 Code.
Access openings shall be provided with doors that shall be kept closed and
locked.
