On or about June 3, 2003, at least four employees were working in trench.
Employees were
required to enter into an existing pipe (36-inch diameter) to work inside
the pipe.
Entry(ies) made into the 36-inch diameter water pipeline, for the purpose
of construction of the
pipeline, without assuring acceptable atmospheric conditions for all
periods
of entry; providing
appropriate respiratory and retrieval equipment in the event of an
emergency; and assuring a
common communication system for monitoring the need for emergency rescue.
Among other methods, recognized and feasible means of abatement to correct
this hazard
include, but are not limited to:
