Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The
employer did not
furnish to each of his employees employment and a place of employment
which were free
from recognized hazards that were causing or likely to cause death or
serious physical harm
to employees in that employees were exposed to:
(a)On or about the evening of June 30, 2002,  two employees of PERSONNEL
PLUS, while working for Land View Fertilizer, Inc., were exposed to the
structural collapse of the roof of the phosphoric acid  pit. These
employees
were hired to hoist, using a bucket, rope and  pulley, sludge  from the
pit to
an outside bin. The pulley attachment point in the hoisting procedure  was
a
roof truss that was known to be damaged from acid mist exposure and
structurally unfit for weight bearing. During the hoisting operation
several
trusses cracked and fell into the pit.
Note: Abatement certification and supporting documentation are required
for this item.
