Section 5(a)(1) of the Occupational Safety and Health Act of 1970:
The employer did not furnish employment and a place of employment
which were free from recognized hazards that were causing or likely
to cause death or serious physical harm to employees in that
employees were exposed to the sudden collapse of a gantry crane:
a) On or about 4/25/04 an employee was exposed to crushing hazards
in the cab of the Kaverit 60-ton crane due to the structural
failure caused by a sudden stop. The crushing hazards resulted from
the rotation of the bridge beams and movement of the legs. The
failure occured because the crane was not properly designed to the
owner's specifications, standard industry practice, and industry-
accepted engineering principles. The crane lacked sufficient
lateral-load resisting stability as it was designed.
Among other methods, one feasible and acceptable method to correct
the hazard is to design the crane to meet the stored wind load and
structural strength requirements of the American Society of Civil
Engineers' Standard ASCE 7-1998 Minimum Design Loads For Buildings
and Other Structures, Section 6.5.4, Figure 6-1, as referenced by
the Crane Manufacturers Association of America's standard CMAA #
70, Specifications for Top Running Bridge & Gantry Type Multiple
Girder Electrical Overhead Traveling Cranes, Section 3.3.2.1.3.1.
Another feasible and acceptable method to correct the hazard is to
design the crane to meet the wind load requirements stated in the
Port of Morrow's specifications for this crane dated October 3,
2003, Section 1.4.3.G(3).ons
