Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed the hazard of crushing injuries due to the sudden and
unexpected decent of
automotive lifts:
a)  McCaddon Oldsmobile-Cadillac Inc., 2460 48th, Boulder, CO:  Located
in the service area, the automatic latch mechanisms on Gilbarco brand
automotive
lifts #2,5,6,9,10,12, and 13, were disabled. Condition has employees
exposed to
crushing injury should primary failure occur.
Among other methods, one feasible and acceptable abatement method to
correct
this hazard is to ensure that all automatic latch mechanisms are installed
and
adjusted per Gilbarco publication number MDE1941A.  Additionally, Gilbarco
publication MDE1360E has a precautionary statement stating, "Do not operate
lifts with automatic lock leg or latch system disconnected or
malfunctioning. Do
not defeat these systems by modifying, removing parts or disarming the
system
in any way.  Should an accident occur, you may be subject to criminal or
civil
liability because of such actions."

were exposed to crushing injury due to improper automotive lift operation:
a) McCaddon Oldsmobile-Cadillac Inc., 2460 48th, Boulder, CO: The
employer had not provided sufficient training to employees operating
automotive
lifts (18) throughout the vehicle service area.  Condition exposed
employees to
crushing injuries due to improper operation.
b)  The employer had not displayed the operating instructions on Galbarco
automotive lifts in a conspicuous location in each lift area.
c)  The Gilbarco lifts were not permanently marked to show the
manufacturer's
name, rated load capacity, model, and serial number.
Among others, one feasible and acceptable method to correct this hazard is
to
follow the American National Standards Institute for Automotive Lifts -
Safety
Requirements for the Construction, Care, and Use, document B153.1 1990.
1)  Paragraph 7.2.2  states "The owner or employer shall ensure that
operators
of automotive lifts are instructed in the safe use and operation of the
lifts using
the manufacturers instructions and the Automotive Lift Institute
publications,
"Lifting it Right," SM89-1, and "Safety Tips," ST90.
2)  Paragraph 7.2.1 states, the owner/employer shall display the operating
instructions in a conspicuous locations in each lift area.
3)  Paragraph 6.2 states that lifts shall be permanently marked to show the
manufacturer's name, rated load capacity, model, and serial number.

employer had not developed and implemented an inspection and maintenance
program for the Gilbarco brand lifts used in the service area.  Condition
exposed
employees to crushing injury due to lift failure.
Among other methods, one feasible and acceptable abatement method to
correct
this hazard is to follow the 17 check items listed in the  manufacturers
(Gilbarco)
publication, MDE 1604G titled "Inspection and Maintenance Instructions for
All
Gilbarco Swing Rail and Two-Post Fixed Piston Lifts".

Abatement Note: Paragraph 6.1.2 in the Automotive Lift Institute

1994 lists the minimum requirements a preventive maintenance technician
