Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to crushing and amputation injuries during the operation of powdered metal (P/M) presses provided with inadequate barrier guards, and/or readily removable fasteners: a) Molding Room 1 - Bliss P/M molding press #M15, full revolution, 60 ton, had an opening in the top of the barrier guard approximately twenty eight inches (28") wide x twelve inches (12") long and seven inches (7") from the point of operation. FEASIBLE ABATEMENT METHODS: 1. Utilize a fixed barrier guard to prevent entry of hands into the point of operation by reaching through, over, under, or around the guard and remove readily removable fasteners in accordance with ANSI B11.16-2003, section 8.4, Point of Operation Guards. b) Molding Room 2 - Gasbarre P/M molding press #M53, part revolution, 30 ton, fixed barrier guard was not mounted with hardware to minimize misuse or unauthorized removal. FEASIBLE ABATEMENT METHODS: 1. Secure and mount the barrier guard to minimize the possibility of misuse or unauthorized removal in accordance with ANSI B11.16-2003, section 8.3, Guards.
