General-duty citation text
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to crushing and amputation injuries during the operation of powdered metal (P/M) presses provided with inadequate barrier guards, improperly adjusted presence-sensing devices, and/or readily removable fasteners: a) Iron Area - Johnson P/M molding press #3B, full revolution, serial #unknown, 27 ton, had an opening in the top of the barrier guard approximately two inches (2") wide x six inches (6") long and eight inches (8") from the point of operation. The barrier guard was not mounted to the P/M press with hardware to minimize misuse or unauthorized removal. Additionally, readily removable fasteners (wing nuts) were used to construct the barrier guard. FEASIBLE ABATEMENT METHODS: 1. Utilize a fixed barrier guard to prevent entry of hands into the point of operation by reaching through, over, under, or around the guard and remove readily removable fasteners in accordance with ANSI B11.16-2003, section 8.4, Point of Operation Guards. 2. Secure and mount the barrier guard to minimize the possibility of misuse or unauthorized removal in accordance with ANSI B11.16-2003, section 8.3, Guards. b) Iron Area - Gasbarre P/M molding press #M50, part revolution, serial #7988, 30 ton, had a light curtain presence-sensing device with two inch (2") wide x sixteen inch (16") long gaps between each infrared sensor approximately nine inches (9") from the point of operation. FEASIBLE ABATEMENT METHODS: 1. Utilize a presence-sensing point of operation device capable of detecting the operator's hand in the sensing field in accordance with ANSI B11.16-2003, section 8.5.3, Presence-sensing point of operation device.on
